GSI stands for Guaranteed Standard Issue. As a disability insurance structure, it removes the medical underwriting step entirely. Under standard individual disability insurance, a carrier reviews your health records, may order labs or an attending physician statement, and can decline coverage, add exclusion riders, or raise your premium based on what it finds. Under a GSI program, that review does not happen. Approval is based on eligibility criteria alone. No health exam. No prescription database check. No review of prior treatment or diagnoses.
The tradeoff is access. GSI is only available at specific residency and fellowship programs where a carrier has chosen to make the offer available.

How Guardian’s GSI Program Is Structured
Only a few carriers offer GSI programs at GME programs. Guardian’s version is available at more than 200 residency and fellowship programs nationally, making it one of the largest GSI offerings available to physicians in training. Guardian independently makes the offer available to physicians in training at eligible programs, and no institutional relationship is required on the hospital’s part.
The underlying policy is the ProVider Choice, Guardian’s own-occupation individual disability contract, issued by Berkshire Life Insurance Company of America, a wholly owned Guardian subsidiary. This is an individual policy you own. It is not a group benefit. It does not follow your employer. It does not terminate when training ends.
Who Qualifies
Four conditions generally determine access to the offer.
You must be in active training at a program where Guardian makes the GSI offer available. You must be working full time, defined as at least 30 hours per week, and must have been doing so continuously for at least six months. You must not currently be collecting disability benefits of any kind. You must not have a disqualifying prior disability insurance application with a carrier other than Guardian, as explained in the sequencing section below.
Health history does not factor into the determination. Pre-existing conditions, prior diagnoses, and ongoing treatment are not reviewed. If the conditions above are met, you are approved.
The Enrollment Window
The GSI offer is available during active training and for up to 90 days after graduation. That post-graduation window is a firm deadline and exceptions are rarely, if ever made. After that window closes, medical underwriting is required.
The less visible deadline is the one that arrives without notice. Guardian reviews GSI offers annually and does not guarantee that they will continue to be made going forward. There is no advance warning to residents. The offer simply becomes unavailable for those who have not already enrolled. Because both deadlines reward acting sooner, it helps to understand when to apply.
The Sequencing Rule
This is the eligibility condition most residents discover too late.
If you apply for disability insurance with any carrier other than Guardian after the first nine months of your training start date, the outcome of that application determines your GSI eligibility. A decline, a modification such as an exclusion rider or limited benefit period, or a withdrawn application all disqualify you from the GSI program.
Two exceptions exist. If the outside application happened before, or within the first nine months of your training start date, it does not affect GSI eligibility regardless of the outcome. Guardian accounts for the reality that residents in their early months of training may not yet know the GSI offer exists. And if you applied to another carrier after nine months and received a clean approval with no limitations or exclusions, your GSI eligibility is intact.
Applications submitted before your training at the GSI-eligible program began do not count. What happened before residency stays there. Applications to Guardian at any point during training do not affect GSI eligibility regardless of outcome.
Benefit Amounts
During training, Guardian’s GSI issue limits are tiered by year. Residents in any year before their final year may qualify for up to $5,000 per month. Residents or fellows in their final year of training qualify for up to $8,000 per month. Guardian does not reduce these limits based on any group long-term disability coverage your training program provides.
Many residents enroll at $2,500 per month to keep the premium manageable during training. A graded policy at that benefit level typically runs $50 to $75 per month. Graded vs. Level Premiums
Guardian GSI offers two premium structures. Graded premiums start lower during training and increase incrementally over time. Level premiums are fixed at issuance and do not change with age.
Most residents choose graded premiums. The lower starting cost fits the income reality of residency. Level premiums make more sense when long-term cost predictability is the priority. Neither structure changes the coverage terms or Guardian’s obligation to keep the policy in force. Health changes after issuance have no effect on premiums or coverage.
Growing Coverage After Graduation
The policy you carry through training is usually not the policy you will need as an attending. A resident who qualifies for $5,000 per month during PGY-1 will need significantly more coverage on an attending income.
The Future Increase Option and Benefit Purchase Option riders allow you to increase coverage after graduation without medical underwriting. No health questions. No exam. No possibility of decline based on anything that developed during training or after.
In the first two years after completing training, Guardian allows an increase to $8,000 per month with no financial documentation required. This is the standard first step for every new attending who carried a policy below that level during residency.
Above $8,000, increases require income verification. An employment contract typically satisfies that requirement. How much above $8,000 you can qualify for depends on your attending income and the group long-term disability coverage your new employer provides. The maximum total coverage for a GSI policy is $15,000 per month. Employer group long-term disability coverage alone rarely protects a physician’s income adequately, which is why employer group LTD coverage falls short.
How This Differs from Traditional Disability Insurance
A standard individual disability application involves full medical underwriting. The carrier reviews your records, may require labs, and makes a coverage decision based on what it finds. Residents who go through standard underwriting frequently receive modified offers: exclusion riders for mental health treatment, musculoskeletal conditions, or any condition that was documented during training.
Under GSI, none of that review occurs. The guarantee comes with restricted access: the offer is only available through Guardian’s program and only during the training window.
One structural point carries significant weight. If you go through traditional underwriting first, with a carrier other than Guardian, and receive a decline, modification, or withdraw an application, you lose access to the GSI offer. The sequence cannot be reversed after the fact.
